If you would like to make an appointment, please click the link below, or contact our office by phone.  We strive to bring patients into the office in a timely manner, many times we are able to see you the next business day. 

​If you are unable to keep an appointment, we ask that you kindly ​provide us with​ ​at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.

Financial

Forms of payment accepted by the office are any major credit card or cash.  We also accept care credit if you need help financing a treatment plan, click the link below if you need to apply. 

​Being a Self pay practice, we keep things as affordable as possible.  We are not participating providers with any insurance companies.  Because we do not participate with insurance plans, we are able to offer reasonable cash pay plans.  We will provide you with a receipt complete with treatment codes, which you can submit to your insurance company, and depending on the policy, MAY be eligible for reimbursement or application toward deductible.
Therefore we also, since we are surrounded by so great a cloud of witnesses, let us lay aside every weight, and the sin which so easily ensnares us, and let us run with endurance the race that is set before us,     Hebrews 12:1
